Deep in the execution chamber of Floor 3B, Cathy, the sadomasochistic executioner and punisher of criminals, gives Ray a pistol, trying to get her to turn on Zack. However, Zack is not down for the count yet.

Prison guard Cathy tries to break the vow between Ray and Zack by continuing to torture them with painful punishments. Then Cathy gives Zack her special "medicine" which enhances his urge to kill and causes him to come after Ray with his scythe.

When Ray and Zack finally arrive at Floor B3, they meet the mad prison guard Cathy Ward. There she punishes them with her sadistic traps including an electric chair and poison gas, and they struggle to escape each one and keep their promise to each other.

Amnesia-stricken Ray and serial killer Zack have reluctantly joined together to escape the basement. They stumble onto Floor B4 where they find a room full of headstones, with one seemingly belonging to Ray.

Rachel Gardner wakes up in a basement she knows not where, where a serial killer wrapped in bandages named Zach tries to kill her with a scythe. She escapes, but on the next floor she meets a man called Danny, who says he is her doctor--but as Rachel breathes a sigh of relief, she can't help noticing Danny has strange eyes.
