Dr. Kenzo Tenma risks his life to track down a brutal serial killer, who turns out to be a patient whose life he saved more than nine years earlier and who now has sinister government connections.

Tenma, in searching for information about the 511 Kinderheim orphanage, and Johan's twin sister Anna, who wants to stop her brother herself, are both led by their investigations to a powerful neo-Nazi organization that conspires to establish Johan as their new superleader.

Dr. Kenzo Tenma focuses on the infamous experimental orphange in East Berlin in his search for the killers of a string of murders for which he is the prime suspect.
